From the update 1.4.X of CarPlay iOS, you can change the background of the main menu in some easy steps.
- Launch the Settings app of CarPlay iOS
- Touch the options Background
- Allow CarPlay iOS to browse your pictures
- Touch one of your picture to preview the background

you can change the menu of CarPlay iOS from the files system of iOS.
The background image can be placed at this path :
/var/mobile/carplayios_background.png
But you need to kill CarPlay iOS and relaunch it after the modification of this file.
